Before scheduling your first Pet Check-In, please review and provide the following details. This ensures your pet’s health, safety, and comfort — and allows us to deliver the highest standard of care.

🐶 1. Pet Information

☐ Pet’s full name and species (dog or cat)
☐ Breed, age, and approximate weight
☐ Personality notes (friendly, shy, energetic, cautious, etc.)
☐ Known fears or triggers (thunderstorms, strangers, vacuum, etc.)
☐ Any medical conditions or allergies
☐ Proof of current vaccinations (copies or digital records may be provided before your first visit)
For the safety of all pets and caregivers, all dogs, cats, animals must be up to date on core vaccinations (including rabies). Please present proof during your free initial meet-and-greet.

🍖 2. Feeding & Water Instructions

☐ Location of food, treats, bowls, and feeding schedule
☐ Specific feeding amounts or dietary notes
☐ Water bowl refill location and any special instructions (filtered, bottled, etc.)

🚶‍♀️ 3. Outdoor & Exercise Guidelines

☐ Potty area or preferred walking route
☐ Location of leash, harness, or poop bags
☐ Instructions for rainy or extreme weather days (use covered porch, towel dry, skip walk, etc.)

🐾 4. Indoor Care & Wellness

☐ Favorite toys or comfort items
☐ Litter box location and cleaning instructions (for cats)
☐ Signs of normal behavior or any habits we should know
☐ Temperature or lighting preferences if you’re away overnight

🧽 5. Clean-Up Supplies (for Accidents)

☐ Location of paper towels, pet-safe cleaners, and trash can
☐ Instructions for preferred clean-up methods (carpet, hardwood, tile, etc.)
☐ Pet-safe disinfectant or deodorizer if applicable

🌧️ 6. Rainy Day Instructions

☐ Where to take pet out if raining (covered patio, garage area, etc.)
☐ Towels or wipes location for drying paws and fur
☐ Indoor enrichment suggestions (treat puzzle, toys, or cuddle time)

🏡 7. Home Access & Security

☐ Entry instructions (key, lockbox code, alarm system, gate access, etc.)
☐ Emergency contact (name + phone number)
☐ Vet name, address, and phone number
☐ Backup contact in case of weather or emergency delays

💕 8. Initial Meet-and-Greet

☐ Confirmed free introductory visit date/time
☐ Pet has met their Love Furry Babies caregiver and is comfortable
☐ Vaccination records reviewed or copies provided
☐ Any final notes or special requests shared
